Puppy Application

Thank you for your interest in adopting a Midwest Bernedoodles puppy! This online puppy application will assist us in finding the best Midwest Bernedoodles puppy for you! Please fill out the following application thoroughly and accurately.

After you have submitted the application, we will respond to you within 48 hours.  After your application has been approved, we will request that you submit a $500.00 non-refundable deposit. This will secure your spot for an MWBD puppy or place you on our waiting list. Payment information is available on our “Payment” page (tab under Puppies).

Your $750.00 deposit will be applied to the purchase price of your puppy. If we are unable to provide you with a puppy that meets your criteria within 12 months of receipt of your deposit, we will refund it in full. We consider your application on a first come basis and will secure your spot for a MWBD puppy or place you on our waiting list once your non-refundable $750.00 deposit is received.

  • By answering the following questions we can best place a puppy in your home that suits your family and lifestyle.
  • Midwest Bernedoodles come with a two year guarantee against genetic diseases. Your puppy will receive its first shots, microchip and dewormings at age appropriate times. We also provide lifetime support for our puppies/dogs and will rehome them if needed.

  • I understand that puppies are not made in a factory, but left to the chance of Mother Nature. My preferences may or may not be met within a particular litter. I also understand that temperament is most important to Midwest Bernedoodles when matching puppies to families. I am open-minded and flexible in regards to my puppy's color, markings and gender within a particular litter. Also, I understand that Midwest Bernedoodles allocates puppies and has the right to reserve any puppy for breeding they choose.
